Teams Working Towards This Vision

The Collective Impact project team is comprised of more than ten local consultants that are trained and available to assist organizations and individuals wishing to utilize the Collective Impact Model, a cross-sector approach to solving complex social and environmental issues on a large scale. Transform Rockford recently used a Collective Impact approach on making sense of data on 3rd grade reading. Learn about the results and the Ready to Learn team. Interested in being a trainer? The Rockford Area Case Management Initiative (RACM) provides a community-wide approach to deliver individualized, strength-based, and person-centered support services that helps participants achieve specific, self-defined outcomes leading to self-sufficiency. RACM was developed by a planning team of local nonprofits, supported by Transform Rockford, and is given ongoing strategic support by a Steering Committee whose members hail from over a dozen local human service organizations. The group hosts quarterly events for case managers to help increase knowledge on specialized topics, share information and best practices for the field of case management in general, and provide an opportunity for networking among local case managers. Are you a case manager? Join the initiative!
